The objective of this qualitative study was to identify the knowledge of community health agents working in the Family Health Strategy regarding the social health determinants in their ascribed area. To do this, the focal group technique was used, with sessions registered using a digital voice recorder and camera with the subjects’ authorization. A content analysis of the data was performed. The data were grouped into three thematic categories: The individuals’ lifestyle - socially and culturally determined lifestyles; Social and community networks - level of social cohesion; and Life and work conditions - material conditions required for survival. The identification and analysis of the social health determinants could help to establish appropriate management practices with the purpose to facilitate the decision-making process, form cooperation networks, perform a critical analysis of the health-disease process, and redirect the care provided.
