This article analyzes the initiatives to promote access of the transgender population to the health system in four municipalities of the conurbano bonaerense (Province of Buenos Aires, Argentina) between 2021 and 2023. A qualitative methodological design consisting of in depth interviews with health professionals working in clinics specializing in transgender health care, as well as with decision-makers and technical team members from national, provincial, and municipal government agencies responsible for implementing public policies on health, gender, and sexual diversity, was employed. The article identifies the institutional and social networks that enable or hinder strengthening the work of these clinics. It traces links between the challenges of guaranteeing health care to the transgender population, the processes of decentralization of primary health care policies, and the gradual institutionalization of the gender and sexual diversity agenda in the province of Buenos Aires.
